To ground an assessment we need to work through all five stages and be able to provide detailed responses to at least the first four steps.
This process for grounding an Assessment comes from Sieler (2003), Coaching to the Human Soul (Volume I).

What is being taken care of by making this assessment?
What future concerns do I address by having this assessment?
What purpose is served by this assessment?
How is this assessment working for me?
Step 2. In which particular domain of action?
Be precise in which domain the assessment applies to.
Step 3. According to what standards?
Where did the standards come from?
Are the standards reasonable/appropriate?
Step 4. What true assertions support the assessment?
Be careful of grounding assessments with other assessments (ours or others).
What specific instances/evidence can I point to that support my assessment?
Step 5. What true assertions are there against the assessment?
