The 4-step process of learning from an After action review can be undertaken in all areas of life, as this example shows.

  1. What was supposed to happen?

    As the water was poured from the fish tank over the net, our pet Siamese fighting fish called Domenic was supposed to ‘flow’ into the net.

  2. What actually happened?

    At the last moment Domenic seemed to jump sideways, missing the net and landing in the sink. Right next to the plug hole which he immediately slid into.

  3. Why were there differences?

    Who can ascertain what went through the fish’s mind. Compared to past efforts, this time it was a little rushed.

  4. What can we learn?

    The obvious danger was thought to be a dropped fish, never that he would ‘do a Nemo’ and disappear down a drain. Next time we will fill the sink with enough water so that if the new fish jumps, it can land safely and be caught in the net.
